06/16/11 Prior to loading, electricity changed started on crane breaker.
06/26/11 At 1145, High Stack Temp./Flare Failure Shutdown.
1205 High Stack Again.
Closed Flare Valve down to 50%.
07/21/11 2350: High Liquid Seal Level.
08/05/11 Back to Back ESD’s; High Stack Temp at Flare.

09/17/11 0900 Down.  Vessel Overfill Hignh O2 Alarm.
Alarm Pulled and Reset.
0950 Restarted.
1350 Down Again; Vessel Overfill Alarm Pulled.
1355 Restart and Reset Alarms.
11/30/11 High O2 Shutdown.
02/04/12 1305 – 1355 Bad Weather. High Level Alarm, Flare Pilot Failure shut me down.
04/11/12 0405 – 0419 Low Seal Flare Failure (liquid level).
05/03/12 1620 High O2 Shutdown / Fuel Gas valve went to 0% while in auto / unknown cause / restart at 1630.
05/08/12 0835 Low level seal pot shutdown.
05/25/12 1340-1405 High level liquid seal shutdown.
05/25/12 1635-1650 High level liquid seal shutdown.
07/23/12 1920-2020 Loading Pump Shutdown.
10/17/12 0700 BM & SB calibration on O2 & H2O pressure sensors.
0820 Start Loading Kirby barge T-Bone transfer “25,000 bbls”.
1030 George checked H2O pressure on vapor line found no bad readings.
1035 O2-12.40% / 45.2% FG / H2O psi 16.7 @ 2,423 bbls per hour.
1135 raised bph from 2345 – 2588 / O2-12.44% / 45.5 FG, H2O psi 16.1 – Filled fuel pot @ flare 1/4 till low level.
1230 raised bph from 2360 bph – 2647 bph. No answer to how or why the rate went down.
1240 bph 2584 O2-12.14% psi 17.0.
1440 Flow rate 100% 2850 bbls/hr.
1500 bbls/hr 2834 O2-12.47% / H2O psi 14.7.
03/01/13 0130-0310 Tried to find out why there was no flow. Tankerman did not have valves fully open. 0420-0445 Air Compressore quit working and clsosed all control valves. After fiving it a rest, we resumed. 0520 Shutdown no air pressure. 0750 Restart. After ESD panel repair. 0810 Down. Fuel Gas Valve Bad – Broken. 1120 Restart after repair. 1435-1520 Shutdown to refuel compressor.
05/15/13 0530-0600 Shutdown due to Low Liquid Level at Flare.
05/21/13 1415-1540 Shutdown while swapping from Tebone to Crosstex.
08/06/13 1140-1158 Shutdown due to High Seal Pot Level. Did not resume loading Barge K29014 until 1239 so pressure could be flared from vessel.
09/04/13 0725-0738 High pressure shutdown.
09/07/13 0751-0801 High Pressure Shutdown.
09/12/13 1930-1945 Flare shutdown. Liquid Seal Pot. Drained enough to get alarm to clear and restarted loading.
